“Rooted & Replenished,” “we,” “us,” and “our” refer to Rooted and Replenished LLC.
By accessing our website, submitting information, scheduling an appointment, purchasing or receiving a service, or otherwise interacting with us, you acknowledge and agree to these Terms.
Additional intake forms, informed-consent forms, financial policies, clinical disclosures, privacy notices, and service-specific terms may apply to particular services. Those additional documents are incorporated into these Terms where applicable.
1. About Rooted & Replenished
Rooted & Replenished provides mobile IV hydration and wellness services at appropriate locations, which may include:
Private residences;
Hotels;
Workplaces;
Event venues; and
Other locations approved by Rooted & Replenished.
Service availability, geographic areas, treatment options, eligibility requirements, pricing, provider availability, and appointment times may change.
We may refuse, postpone, modify, discontinue, or cancel a service when reasonably necessary because of:
Safety concerns;
Clinical judgment;
Incomplete or inaccurate information;
Provider availability;
Location or environmental concerns;
Legal or regulatory requirements;
Customer conduct;
Operational limitations; or
Other legitimate business or clinical reasons.
2. Not an Emergency Service
Rooted & Replenished is not an emergency medical service, emergency department, urgent-care facility, or substitute for emergency care.
Do not use our website, email, telephone number, booking system, social-media account, or text-messaging program to request emergency assistance.
Call 911 immediately if you believe you or another person is experiencing a medical emergency.
Possible medical emergencies may include:
Severe difficulty breathing;
Chest pain;
Loss of consciousness;
Signs of stroke;
A severe allergic reaction;
Uncontrolled bleeding;
Seizure;
Severe confusion; or
Any condition requiring immediate medical attention.
3. General Medical and Wellness Information

General information from Rooted & Replenished:

Does not establish a provider-patient relationship by itself.
A clinical relationship, when applicable, begins only after required intake, evaluation, consent, and acceptance procedures have been completed.
Treatment recommendations and eligibility determinations may depend on factors including:

Allergies;
Medications and supplements;
Pregnancy or breastfeeding status when relevant;
Previous treatment reactions;

4. Customer Eligibility and Responsibilities
You agree to provide complete, truthful, accurate, and current information when scheduling or receiving services.
You agree to inform the treating provider about information that may affect your safety, including relevant:

Allergies;
Medications;
Supplements;
Pregnancy or breastfeeding status;
Prior adverse reactions;
Recent illnesses;
Injuries;
Procedures;
Hospitalizations; and
Changes in symptoms or health status.
You agree to follow reasonable pre-service, treatment, and post-service instructions.
We may decline to provide a service when:
Required information is incomplete;
Information appears inaccurate;
A requested treatment may be inappropriate;
A provider determines that evaluation by another medical professional is advisable;
The service environment is unsafe or unsuitable;
The customer is impaired or unable to provide valid consent;
Required consent cannot be obtained; or
Providing the service would violate law, regulation, licensing requirements, or clinical standards.
Services for minors require participation and authorization from a parent or legal guardian as required by law and our policies.
5. Appointments and Scheduling
Appointment requests are subject to:
Provider availability;
Geographic availability;
Eligibility screening;
Location approval;
Travel requirements;
Completion of required intake forms;
Completion of required consent forms; and
Final confirmation by Rooted & Replenished.
Submitting a booking request does not guarantee that the appointment has been accepted.
You are responsible for providing accurate contact, location, access, and scheduling information.
You agree to provide a location that is reasonably safe, sanitary, private, and suitable for the requested service.
We may reschedule or cancel an appointment because of weather, traffic, provider illness, emergencies, unsafe conditions, incomplete intake requirements, or other circumstances outside our reasonable control.
6. Pricing, Payments, Deposits, and Refunds
Current pricing, travel charges, event requirements, deposits, cancellation fees, and refund terms will be disclosed during booking, in an applicable financial policy, or before services are provided.
You agree to pay all charges that you authorize.
You authorize Rooted & Replenished and its payment processors to charge the payment method you provide for:
Services received;
Approved treatment selections;
Approved add-ons;
Applicable travel fees;
Deposits;
Cancellation or no-show charges disclosed in advance; and
Other amounts authorized by you.
Unless otherwise stated in writing, estimates are not guaranteed when the requested service, location, travel requirements, eligibility, or treatment selection changes.
Refund eligibility will be determined under the financial or cancellation policy presented to you and applicable law.
7. Treatment Consent and Risks
Before receiving a clinical or wellness service, you may be required to review and sign an informed-consent document describing:
The proposed treatment;
Expected benefits;
Material risks;
Possible side effects;
Alternatives;
Contraindications;
Post-treatment instructions; and
Circumstances requiring additional care.
These Terms do not replace an informed-consent document.
You may ask questions before consenting to treatment. You may decline or withdraw consent before a treatment is administered, subject to charges or cancellation terms already incurred and applicable law.
8. HIPAA and Health-Information Privacy
Rooted & Replenished is committed to protecting the privacy and security of health information.
To the extent Rooted & Replenished is acting as a healthcare provider, covered entity, or business associate subject to HIPAA, we maintain privacy and security practices designed to comply with applicable HIPAA Privacy, Security, and Breach Notification Rule requirements.
When HIPAA applies, we maintain reasonable and appropriate administrative, physical, and technical safeguards designed to protect protected health information and electronic protected health information.
We use and disclose protected health information only as permitted or required by applicable law. We require applicable service providers that create, receive, maintain, or transmit protected health information on our behalf to enter into appropriate business associate agreements when legally required.

A. Program name
The messaging program is operated by Rooted and Replenished LLC, doing business as Rooted & Replenished.

Message and data rates may apply.
Your wireless carrier may charge you for sending or receiving SMS or MMS messages. Rooted & Replenished does not control carrier charges.
F. Opting out
You may opt out of text messages at any time by replying STOP.

We may send one final nonmarketing message confirming that your opt-out request has been processed.

11. Sensitive Information and Text Messages
Ordinary SMS and MMS messages may not be encrypted or fully secure.
Do not use text messages to send:
Payment-card numbers;
Social Security numbers;
Photographs of identification documents;
Detailed medical histories;
Complete medical records; or
Other highly sensitive information,
unless Rooted & Replenished specifically instructs you to use an approved method.
We may send secure intake links, scheduling information, payment links, general instructions, arrival updates, and general follow-up information by text message.
Do not use text messaging to request emergency assistance. Call 911 in an emergency.
12. Website Use
You may use our website only for lawful purposes.
You may not:
Attempt to gain unauthorized access to the website or related systems;
Interfere with website security or functionality;
Introduce viruses, malware, or harmful code;
Attempt to bypass security controls;
Submit false, misleading, fraudulent, or impersonated information;
Use the website to harass or threaten another person;
Use automated tools to scrape or copy website content without permission;
